We report a rare occurrence of primary osteosarcoma of the urinary bladder in a patient originally under surveillance for superficial urothelial

bladder carcinoma, pT1NoMo G II. This multimorbid 74-year-old patient had been treated for intermittent recurrence of urothelial carcinoma

for 13 years and had been unsuitable for radical cystectomy. The patient underwent systemic chemotherapy for early multiple recurrence of

carcinoma in 1997; due to further recurrence, radiotherapy to the pelvic area was delivered. Due to further recurrences with low grading, the patient received repeated local chemotherapy lavage with mitomycin monthly, 12 times in 2003, and with epirubicin monthly, 12 times in 2005.

In the twelfth year of observation, the recurrence of bladder cancer was managed with endoscopic resection. The histological result established

the diagnosis: osteosarcoma of the urinary bladder, G II. The subsequent two-year follow-up showed no recurrence of osteosarcoma.

Keywords: osteosarcoma, urinary bladder, transurethral resection
